Cellulite has a reputation for being almost impossible to resolve. The stubborn dimples remain despite exercise or a healthy diet, leaving many people looking for another option to smooth their skin. If you’re one of the 90 percent of women who are looking for a cellulite solution, you have probably experienced frustration at the lack of results. The truth is, cellulite will not resolve or reduce using natural methods or even creams or other topical treatments, because the cause of the issue lies below the skin.

At Cheng Plastic Surgery, we now offer two very successful treatment options: Cellfina, a non-surgical microblading treatment, and Qwo, an injectable for treating cellulite in the buttocks. Both are minimally invasive.

What is cellulite?

There are long, fibrous cords just below the skin. These cords, also known as septae (plural for septum, from the Latin, meaning fence or enclosure) run vertically and tether the skin to the muscle beneath. As fat cells accumulate between the cords, they push up against the skin while the cords pull down—the reason behind the uneven, dimpled look to the skin.

Cellulite has traditionally been one of the hardest problems to treat. Unfortunately, it does not discriminate. Even a very fit, healthy person can develop these irritating and unsightly dimples or uneven waves in the skin. We finally have two very good treatments to address the unsightly lumps and bumps of cellulite.

Vertical cords connect the muscle to the skin, and fat cells lie in between. These vertical cords run through the fat tissues. As fat cells accumulate between the cords, they push up against the skin while the cords pull down. As you age and your skin becomes more lax, the bands tighten and fat compartments bulge. The effects of this can be seen in dimpled, uneven skin.

Until now, no matter how hard you work out, eat right, or apply special creams, nothing was going to impact those vertical cords.

How does Cellfina work?

Cellfina treats the underlying cause of cellulite, which are the bands of fibrous septae that are contained in fat, but Cellfina uses subdermal undermining (separating the different types of tissue under the skin) to improve the surface texture.

Cellfina uses a non-thermal, non-energy-based method to treat the fibrous septae. A precision-guided platform is used to access and release these bands.

What is the Cellfina procedure?

This treatment is minimally invasive and performed under local anesthesia right in our office. It is a single-treatment procedure, so you can achieve virtually all the results you need in one session.

A local anesthetic is administered before your treatment. Skill on the part of your surgeon is required for the best results. The fibrous bands have to be selected and targeted for treatment, and your doctor will ordinarily “draw” the desired treatment area onto your skin.

Is Cellfina painful?

The Cellfina procedure itself takes about 45 minutes to an hour and is done under local anesthesia. Tiny incisions are made to release the bands just under the skin. You will feel some suction from the Cellfina device as it cuts the cords and disconnects them from the skin. The sensation of suction comes from the fact that these cords have a certain elasticity, so it is a bit like cutting rubber bands away from your skin. When they are cut, the tautness is released, and the skin relaxes. The incisions are small enough that no stitches are required, and you can go home the same day.

What is the recovery from Cellfina?

You will go home with dressings on the incisions, and you will wear a compression garment for two weeks following the procedure. Cellfina provides immediate results.

No recovery time is required after Cellfina, but you may experience some tenderness and bruising for a period of weeks. These effects are usually minor and should not interfere with work or other activities. If necessary, you can take non-prescription medication to ease your discomfort.

What can I expect during a Qwo procedure?

Qwo is injected into the bottom in specific areas to address and treat the fibrous bands in the fat, the underlying cause of cellulite. Before the injection process begins, your surgeon will select the areas targeted for treatment, asking you to stand relaxed, with your feet apart. The dimples selected for treatment will be marked with a dot and circled on the skin.

You will lie face down while Qwo is injected into the fat of each circled area, treating a maximum of 6-12 dimples per buttock. The procedure takes about fifteen minutes. You will do two more injection treatments to follow this procedure, each treatment being done three weeks apart.

The final results are evaluated 28 days after the third injection treatment. Qwo results can be expected to last for at least a year, possibly longer.

Are Qwo injections painful?

The injection is not described as painful for patients, with no need for a numbing agent before the procedure. However, most patients have bruising or mild to moderate soreness afterward, with the soreness feeling as if you had a good workout. This will be resolved in a few days. The bruising is usually more significant after the first injection treatment.

Practice Policy Update Regarding COVID-19

We are open!

We cannot wait to see all of our amazing patients in our office! We are currently working with a more limited schedule than before, so please be patient as we navigate through this, together.

There are a few key changes we’ve made to our appointment protocol to ensure we are maintaining social distancing and following strict COVID-19 health and safety protocols - for you AND for us.

  1. Call our office from your car once you’ve arrived for your appointment. A staff member will come out to greet you, take your temperature and conduct a short COVID-19 health screening to make sure you are in good health prior to entering the building. Our doors will remain locked during normal business hours to ensure only those who have been screened can enter.
  2. Please arrive at your appointment wearing a mask or protective face covering. We will not be able to bring you inside the office without one.
  3. We will not have more than one patient in the lobby at a time.
  4. You will be greeted by Dr. Elbert or Dr. Jacqueline wearing their Personal Protective Equipment deemed necessary by your specific treatment. (N95 mask, gloves, face shield or protective eyewear, etc.) We ask that you leave your mask on during the procedure, and only take it off if necessary to complete the treatment.
  5. All necessary transactions will be done in the room your treatment is being performed. This will avoid unnecessary back ups in the lobby.
  6. You will then be walked out through the back door of the office. This is the best way to keep all staff and patients socially distanced while maintaining one steady flow of traffic throughout the building.

And of course, all rooms will be heavily disinfected immediately after you leave just as we have always done prior to the pandemic. Each room is equipped with an air filter as an added safety measure. We will place a major focus on high-contact areas such as chairs, doorknobs, counter tops, mirrors and more.

We are still continuing to conduct Virtual Consultations if you prefer a quicker and no-contact experience.

For the safety of our patients and staff, we ask that you reschedule your appointment and do not enter our office with any of the following conditions:

  1. You have tested positive for COVID-19 in the last 14 days.
  2. You have been in close contact with someone who has tested positive for COVID-19 in the last 14 days.
  3. You traveled more than 150 miles or have been in close contact with someone who has traveled in the last 10 days.
  4. You are experiencing any of the following symptoms:
    • fever or chills
    • cough
    • shortness of breath
    • fatigue
    • muscle or body aches
    • headaches
    • sore throat
    • loss of taste or smell
    • nausea, diarrhea, or vomiting

Concerned about getting the Moderna vaccine after having recently received filler treatment? In some studies, it has been observed that a small percentage of the vaccinated groups experienced localized inflammation (swelling) after receiving the Moderna COVID-19 Vaccine. In said cases, all studied reactions were temporary immune responses. It has been suggested that patients refrain from getting filler two weeks before and after the vaccine.
